Steps:
1. File religious exemption with school and clinical site. Learn more here or here.
2. Once denied send letter with charge of religious discrimination. See template below.
3. File complaint with Indiana Civil Rights Commission including: Dates and documentation of exemption, denial, charge of discrimination.
4. Talk with an attorney to determine if litigation is an option.

Discrimination Charge Template: 

You may elect to send this letter to both the school and clinical site. It is important to note that hospitals are likely to claim that they are exempt from this due to federal regulations, but you may be able to counter the argument following the "spirit of" both state and federal law to protect students religious freedom equally to that of an employees.
To Whom it May Concern,

My religious exemption to vaccinations was denied on (DATE). Denying access to clinical sites based on my religious beliefs is discriminatory, a violation of my religious freedom, causing me great distress, and jeopardizing my future career.

Indiana State HEA 1001, 2022 provides exemptions for students. Please see below. 

"Sec. 2. As used in this chapter, "employee" means an individual
who works for an employer on a full-time or part-time basis, either
paid or unpaid. The term includes:
(1) an independent contractor;
(2) a subcontractor; and
(3) a student who works as a trainee or an intern."

I am aware of the federal mandate through CMS. Despite the mandate facilities are still obligated to abide by Title VII of the Civil Rights Act granting religious exemptions for staff members.  I have secured an exemption through my employer who is a provider of Medicare/Medicaid services. Aside from discrimination and harassment, there is no reason that a student should not be allowed to practice their religious beliefs as any other member of staff.
