Vendor Registration

Vendor Registration form.

Thank you for showing interest in being a vendor at our first annual fundraiser & family picnic!

If you feel the goals of your organization align with Medical Liberty we can't wait to see you.

All vendors are expected to make a minimum donation of $50 once registration is approved.

The donation includes:

Table & 2 chairs at event (set up starts at 11:30 am day of)
Placement of your logo with a link to your website on our picnic page
Up to 100 pieces of literature or samples placed in "goodie bags" for families in attendance (Literature must be received by Aug 30th)
Lunch for 1, additional donation of $5 each can be made for up to 3 attendees lunches

Virtual Vendors suggested donation: $25.

Up to 100 pieces of literature or samples placed in "goodie bags" for families in attendance (Literature must be received by Aug 31th)

Once your registration is confirmed we will follow up requesting a logo for the website and provide you with a link for donation.
